Runners and hikers who preemptively apply a single layer of surgical paper tape to their known hot spots reduce their risk of developing foot blisters by 40 percent. The intervention requires no specialized gear, costs less than a dollar per roll, and fundamentally changes how endurance athletes manage the most common injury on the trail.
The standard advice for blister management has long relied on thick padding, heavy lubricants, or moisture-blocking powders. Yet, despite 40 years of medical research into these methods, clinical evidence supporting their efficacy remained remarkably thin.
The breakthrough came from the Atacama, Gobi, Madagascar, and Jordan deserts in 2014. Dr. Grant Lipman, an emergency medicine physician at Stanford Health Care, spent 10 years treating ultramarathoners who were running up to 50 miles a day.
“I started noticing just how trashed people's feet will become,” Lipman noted. “What I kept hearing was, 'Doctor, I'd be doing so well, if only for my feet.' Their feet were getting decimated.”
To test a new approach, Lipman and his colleagues designed the Pre-TAPED II trial, published in the Clinical Journal of Sport Medicine in 2016. The study followed 128 runners competing in a 155-mile, six-stage ultramarathon.
Medical assistants applied standard paper tape—commonly known as surgical tape—to the runners' blister-prone areas before the race. If a runner had no history of blisters, the tape was applied to a randomly selected location on the foot. The untaped areas of the same foot served as the control.
The results were definitive. Out of the 128 participants, 98 runners did not develop any blisters underneath the taped areas. In contrast, 81 of those same runners developed blisters in the untaped zones of their feet.

“It's kind of a ridiculously cheap, easy method of blister prevention,” Lipman said following the study's publication. “You can get it anywhere. A little roll costs about 69 cents, and that should last a year or two.”
The mechanism behind the tape's success lies in understanding how blisters actually form. They are not caused by simple surface rubbing, but by shear deformation—the structural separation of cell layers beneath the skin's surface when the foot stops but the skeleton continues moving forward.
“The paper tape itself is very thin and it's very smooth,” Lipman explained. “We theorized that because it's so smooth, it deceases friction and rubbing and shear stress on those sensitive areas just underneath the tape.”
Crucially, the tape's mild adhesive quality is a feature, not a bug. When the tape does eventually peel off, it lacks the tackiness to tear the underlying skin, preventing a closed blister from becoming a painful open wound.
This targeted approach represents a refinement of earlier, failed experiments. In the initial Pre-TAPED I study, researchers applied paper tape to all the digits of the foot, regardless of whether they were prone to blisters. That broad application showed no significant benefit compared to an untaped foot, likely because wrapping every toe altered foot mechanics and moisture retention.[1][2]
Traditional methods have often introduced their own complications. A 1998 study examining the use of antiperspirants on the feet of military cadets found that while it reduced blister formation, 57 percent of the cadets reported severe skin irritation.
Other interventions, such as specialized blister-prevention patches, have shown efficacy in clinical settings but cost exponentially more than a standard roll of surgical tape, making them less practical for daily training or long-distance thru-hikes.
Podiatrists note that while tape reduces shear stress, it cannot compensate for fundamentally poor biomechanics or ill-fitting footwear. A shoe that is too tight will still compress the toes, and a shoe that is too loose will generate friction that overwhelms the tape's smooth surface.[2]
For the best results, the tape must be applied smoothly, in a single layer, before the skin becomes sweaty or dirty. Once a hot spot has progressed to a full fluid-filled blister, paper tape alone lacks the structural thickness to offload pressure, at which point traditional moleskin or hydrocolloid pads become necessary.[2]
