The US Food and Drug Administration has officially expanded the approval of roflumilast cream 0.3% to treat plaque psoriasis in children as young as two years old. Marketed under the brand name Zoryve by Arcutis Biotherapeutics, the medication is now the first and only once-daily, steroid-free topical therapy authorized for this specific preschool-aged demographic.[1][5]
The regulatory decision marks a significant milestone in pediatric dermatology, addressing a longstanding treatment gap for early childhood psoriasis. Previously, the topical phosphodiesterase-4 (PDE4) inhibitor was only approved for adults and children aged six and older, leaving families of younger children with fewer safe options.[2][3]
Plaque psoriasis is a chronic, immune-mediated inflammatory skin disease that affects nearly 9 million individuals in the United States, and it frequently begins during early childhood. The condition presents with itchy, scaling, red, or discolored plaques that can appear anywhere on the body, causing significant discomfort and distress.[1]
In young children, the disease poses unique clinical challenges. Psoriatic lesions in toddlers frequently develop on highly sensitive areas, including the face, genitals, and intertriginous regions—the medical term for skin folds where two surfaces rub together, such as the armpits or groin.[3][4]
Historically, dermatologists and parents have relied heavily on topical corticosteroids to manage these flare-ups. However, steroids come with severe limitations in pediatric care. Prolonged use of topical steroids on thin, sensitive skin can lead to skin atrophy, systemic absorption, and other adverse effects, meaning they cannot be used continuously or applied to delicate areas.[2][3]
Roflumilast offers a fundamentally different approach by targeting the underlying intracellular mechanisms of inflammation without the risks associated with steroids. It is a highly selective inhibitor of PDE4, an enzyme that plays a critical role in the body's inflammatory response.[1]
By inhibiting PDE4 within the skin cells, the cream suppresses the overproduction of pro-inflammatory mediators and helps rebalance the cutaneous immune response. Because it does not thin the skin or disrupt the endocrine system, the FDA has placed no restrictions on the duration of its use or the body surface areas where it can be applied.[1][3]

Beyond the initial four-week trial, supportive long-term safety data was gathered through an open-label extension study, ARQ-151-306. This phase tracked patients in the two-to-five age bracket for up to 24 weeks of continuous treatment to ensure no delayed adverse effects emerged.[3][4]
The results demonstrated a consistent and favorable long-term safety profile, with efficacy persisting across the study period. Investigators noted that the outcomes in toddlers were generally consistent with the pivotal DERMIS-1 and DERMIS-2 Phase 3 trials that originally secured the drug's approval for older populations.[3][4]
For families managing a toddler with psoriasis, the practical implications of a steroid-free, unrestricted cream are profound. Caregivers no longer have to carefully cycle treatments on and off to avoid skin damage, nor do they have to avoid treating the face or skin folds where the disease is often most uncomfortable.[2][5]
The formulation of the cream itself was also designed with pediatric sensitivities in mind. It is manufactured without common sensitizing excipients such as propylene glycol, polyethylene glycol, ethanol, or artificial fragrances, which can sting or irritate already compromised skin barriers.[4]
While the approval is a breakthrough, clinicians note that roflumilast is strictly for topical use and must not be used ophthalmically, orally, or intravaginally. As with any newly expanded indication, researchers will continue to monitor real-world data as the drug reaches a broader population of preschool-aged children.[3]
The authorization represents the seventh FDA approval for the roflumilast franchise within four years, underscoring a broader shift in dermatology toward advanced, targeted non-steroidal topicals.
