It does not fit well when practitioners have compromised knees or shoulders, given the 5.5 per 1,000-hour injury rate heavily concentrated in those specific joints.

Like high-intensity interval training (HIIT) on a stationary bike, combat sports force the heart rate into near-maximal zones before demanding rapid recovery. But where a stationary bike isolates the cardiovascular system from impact, martial arts inextricably link metabolic conditioning with musculoskeletal stress. For adults turning to combat sports for fitness, the choice between a striking discipline and a grappling discipline is not merely a matter of aesthetic preference. It is a decision about which metabolic engine to build and which joints to load.[7]

The two dominant paradigms for adult martial arts fitness are striking—exemplified by Muay Thai and kickboxing—and grappling, dominated by Brazilian Jiu-Jitsu (BJJ). Both offer intense, full-body conditioning, but they achieve it through fundamentally different physiological pathways. Striking relies heavily on the ATP-PC and glycolytic energy systems for explosive ballistic movements, followed by aerobic recovery. Grappling, conversely, demands sustained isometric contractions and anaerobic bursts as practitioners resist an opponent's body weight.[7]

The metabolic engine of striking is remarkably efficient at sustained caloric burn. According to the Centers for Disease Control and Prevention, moderate-intensity physical activity spans from 3.0 to 6.0 Metabolic Equivalents of Task (METs). Vigorous combat sports, such as kickboxing and karate, routinely reach 10.3 METs. This means a practitioner is expending more than ten times the energy they would while sitting quietly. The constant bouncing, pivoting, and explosive extension of the limbs create a systemic cardiovascular demand that mirrors elite track workouts.[3][5]

Grappling presents a different metabolic profile. While the sustained aerobic MET value may be lower during technical drilling, live sparring—known as rolling—demands intense isometric contractions that spike the heart rate anaerobically. A 2013 study analyzing the caloric cost of various martial arts found that while striking arts like Kyokushin karate averaged 333.3 kilocalories per hour, grappling arts like jujutsu averaged a lower absolute caloric cost of 195.1 kilocalories per hour. When evaluating the safety of these disciplines, context matters. While combat sports carry inherent risks, they often result in fewer severe traumas than traditional boxing. "Yes, you're more likely to get injured if you're participating in mixed martial arts, but the injury severity is less overall than boxing," explained Dr. Shelby Karpman, a sports medicine physician at the Glen Sather Sports Medicine Clinic, following a decade-long review of fight data. "Most of the blood you see in mixed martial arts is from bloody noses or facial cuts; it doesn't tend to be as severe but looks a lot worse than it actually is."

Within the martial arts themselves, the musculoskeletal trade-offs are distinct. A 2025 study published in BMJ Open Sport & Exercise Medicine tracked 881 Brazilian Jiu-Jitsu practitioners and found an injury incidence of 5.5 per 1,000 hours of training. The vast majority of these injuries—89 percent—occurred during training, with 79 percent happening during free sparring. Because grappling relies on joint manipulation and leverage, the knees and shoulders bore the brunt of the damage, accounting for 25 percent and 13 percent of injuries, respectively.[1]

Striking disciplines distribute their wear and tear differently. A foundational study in the British Journal of Sports Medicine examining Muay Thai kickboxing found that beginners experienced an annual injury rate of 13.5 per 1,000 participants, which dropped significantly to 2.4 per 1,000 for amateurs as their technique improved. Unlike grappling, where joint locks cause structural strain, striking injuries are predominantly soft tissue trauma and fractures to the lower extremities, driven by the repetitive ballistic impact of kicking and checking kicks.[2]

Ultimately, the decision between striking and grappling comes down to aligning a practitioner's physical baseline with the sport's specific demands. Those seeking maximum cardiovascular endurance and calorie burn may find striking more efficient, provided their ankles and shins can handle the impact. Conversely, those prioritizing functional strength and isometric endurance may thrive in grappling, provided their knees and shoulders possess the stability required to withstand heavy rotational forces.[7]
